Job Description:
Position Summary/Position Under the direction of the Behavioral
Health & Care Management (BH&CM) leadership the Supervisor,
BH&CM Support Services is responsible for assisting in the
program development and implementation of the care management
operations in BH&CM Support Services. The Supervisor is
responsible for overseeing daily operations of care management,
coordination of care, and continuity of care for IEHP members
utilizing the Housing Program, Long-Term Services and Supports, or
receiving County wrap around services. In addition, the Supervisor
provides oversight and supervision of the integrated services for
IEHP members with complex conditions, as well as administrative and
operational oversight. This position engages the care team members,
working collaboratively to build and maintain high levels of
engagement, as well as excellent member experience.
Major Functions (Duties and Responsibilities) 1. Develop effective
relationships with co-workers, direct reports, and community
partners, modeling excellent interpersonal skills and collaboration
in working toward job duties and goals.2. Model the importance of
member experience and engagement and engage with other leaders in
BH&CM and at IEHP in developing and implementing member and
Team Member experience improvement activities. 3. Communicate,
formally and informally, the nature and importance of Support
Services in overall health to the IEHP BH&CM department, as
well as the larger organization. Engage with other IEHP leaders and
community partners in developing and implementing large scale
public health education on Support Services. 4. Work closely with
the BH&CM leadership to develop effective Support Services care
management principles, processes, and workflows as well as monitor
care management services to ensure compliance with all IEHP, state
and federal regulatory requirements.5. Provide leadership, support,
and administrative supervision of Team Members, to ensure delivery
of high quality, cost effective services to members.6. Proactively
recognize problematic or risk areas and help develop effective and
efficient solutions in conjunction with BH&CM.7. Collaborates
with BH&CM providers, IEHP members and other departments
regarding Support Services concerns.8. Professionally and
collaboratively Interface with Utilization Management Department,
Member Services, and Care Management Department to resolve issues
and establish workflow processes to ensure delivery of high-quality
services.9. Become proficient in all electronic medical management
systems (e.g. MedHok, HSP, Super Search and Web Portal) to assist
in training of new staff members.10. Professionally represent the
department at selected IEHP meetings including external events as
necessary.11.Collaborate with BH Supervisors of Integrated Teams
(Inpatient, Special Pops, Health Homes) on the tracking of HEDIS
measures and implement interventions to meet goals.12. Provide
exemplary leadership for all assigned BH&CM team members
ensuring team members are given every opportunity to succeed in
their positions and are supervised in accordance with Department
and Human Resources (HR) policies.13. Identify deficits within the
department and actively find solutions, including but not limited
to providing trainings14. Provide supervision as necessary,
consulting on high-risk cases and providing direction to the
BH&CM Department.15. Review and update departmental policies
and procedures as necessary.16. Ability to evaluate current
programs for efficiency and program outcomes by analyzing specific
data and providing suggested improvements.17. Refers appropriately
to and maximizes use of community services, support programs,
internal/external resources available to Members including Embedded
Case Management, Health Homes, Palliative Medicine, Behavioral
Health, Transplant programs, pain management, Housing services,
Long-Term Services and Supports, etc. 18. Ability to perform
comprehensive assessment of patients and create a care plan to
address medical, psychosocial and financial concerns or issues
facing the member. 19. Collaborates with embedded BH&CM Team,
Pharmacists, and Interdisciplinary teams.20. Solves complex
problems and/or conducts complex analyses impacting
transitions/readmissions.21. Assesses the effectiveness and quality
of services provided related to functional, and financial impact,
and outcomes.22. Facilitates case conferences to ensure safe
transitions of care as Member moves through various stages of
treatment.23. Educates and supports Members to assist them in the
best decision making for their health and wellness.
Supervisory ResponsibilitiesLeader: Administers Hires,
Terminations, and Performance Reviews
Experience Qualifications Three to five (3 - 5) years of experience
in an HMO or experience working in a Social Service setting
required. Minimum of three (3) years of previous
supervision/management experience required.
Education Qualifications Master's degree from an accredited
institution required.
Professional Licenses Possession of an active, unrestricted, and
unencumbered license in Clinical Social Work - Licensed Clinical
Social Work (LCSW).
Drivers License RequiredYes, must have a valid California Driver's
license and valid automobile insurance. Must qualify and maintain
driving record to drive company vehicles based on IEHP insurance
standards of no more than three (3) points.
Knowledge Requirement Deep understanding and knowledge of services
for members experiencing homelessness, long-term services and
supports, mental health and substance use conditions, including
both acute and chronic management of. Skillful in informally and
formally sharing expertise. Awareness of the impact of unmitigated
bias and judgement on health; commitment to addressing both. Must
have skills and resilience to tolerate a moderate level of change
and development around new models of care and care management
practices. Knowledgeable and skilled in evidenced based
communication such as Motivational Interviewing, or similar
empathy-based communication strategies. Understanding of and
sensitivity to multi-cultural community. Knowledge of whole health
and integrated principles and practices is preferred.
Skills Requirement Bilingual (English/Spanish) preferred.
Proficient in the use of Microcomputer applications (e.g. Microsoft
Word and Excel, Power Point) for use in all aspects of an office
environment and ability to learn new computer systems. Highly
skilled interpersonally, with excellent relationship skills. Highly
skilled in interpersonal communication, including resolving
conflict with co-workers. Possess strong organizational skills.
Abilities Requirement Must be able to work as a member of a team,
executing job duties and making skillful decisions within one's
scope. Ability to function independently. Able to sufficiently
engage members and providers.

of our organization. Starting Salary: $85,841.60 - $109,449.60 Pay
