Job Description POSITION SUMMARY: The Clinical Manager shall
have the day-to-day responsibility for coordination of patient care
services and shall be responsible for all activities relevant to
the patient care services furnished for team. They will ensure the
quality and adequacy of services provided and supervision of staff.
Assumes lead responsibility for patient advocacy; and carries out
the organization's purposes in a manner consistent with the goals,
objectives, and established policies. REPORTS TO: Director of
Patient Care Services SUPERVISES: Direct Patient Care Staff
QUALIFICATIONS: Education: A registered nurse with a minimum of
Associate Nursing Degree, with 3-5 years in a hospice or home
health agency, at least 1- 2 years supervisory experience managing
an interdisciplinary team preferred. BSN preferred Credentials:
Current, active RN license within the State. Certification in
hospice, Home Health desired. Core Competencies: Sufficient
background knowledge and expertise in clinical decision-making, and
management of clinical staff. Demonstrated competency in
performance of professional nursing functions. Knowledge of
applicable laws, regulations, and standards. Excellent
communication skills. Ability to organize, delegate, supervise and
evaluate staff. Understanding of the roles and responsibilities of
the interdisciplinary team. Possesses a sympathetic attitude toward
the caring for the patient and demonstrates positive communication
skills in interacting with other members of the team. Critical
thinking skills and creative resolutions for day-to-day operational
issues. Other: Valid driver---s license, active nursing license,
and auto insurance. FUNCTIONS AND RESPONSIBILITIES 1. Daily
oversight for the overall clinical interdisciplinary team. 2. Serve
as a back-up for the DPCS when there are coverage needs. 3.
Responsible as first point of contact for field clinicians,
patients and their families. 4. Assist to ensure overall compliance
with applicable laws, regulations and standards related to patient
care. 5. Work with Director of Patient Care Services to identify
staffing and staff development needs. 6. Facilities
Interdisciplinary Group Meetings (IDG) and team meetings. Integrate
input of IDG when making clinical or administrative decisions such
as coverage for requested treatments and therapies, change in level
of care, and patient discharge. 7. Assist with triaging patient---s
needs and lead staff in clinical problem-solving as needed; assign
clinicians to patients; oversee daily assignments of clinician
visits that need to be covered. 8. Provide direct supervision of
field staff for new hires and on going training and support for
current staff as needed 9. Responsible for the oversight of the
assessment and evaluation, intervention and documentation of
patient and family needs. 11. Investigates problems, grievances and
complaints and take corrective action. 12. Collaborates with the
DPCS with implementation of initiatives and maintenance of Quality
Assessment and Performance Improvement program expectations. 13.
Responsible for managing clinician---s schedules during regular and
after hours. 14. Ensures that mandatory compliance education and
performance evaluations are completed in a timely manner 15.
Ensures the accurate and timely completion of Medical Records
including compliance with applicable laws and regulations. 16. Acts
as a patient advocate. 17. Maintains continuing education through
academic studies, seminars, and workshops. 18. Participates in
hiring and retention of employees 19. Assures that all clinical
decision-making follows ethical guidelines. 20. Demonstrates
positive leadership skills through effective communication and
personnel selection, development and motivation. 21. All other
tasks and duties deemed necessary and appropriate.
