Job Description:
A company is looking for a Community Health Worker.
Key Responsibilities
Provide culturally appropriate complex care management in a primary
care setting
Assist patients in overcoming barriers and navigating the
healthcare system
Support patients in making meaningful progress toward
health-related goals over a 6-month period
Required Qualifications, Training, and Education
High school diploma or GED preferred
Valid driver's license and clean driving record if driving is
required
Two years of experience in a social service or healthcare setting
preferred
Must be a resident of the community served with knowledge of local
resources
Basic computer skills preferred
