Job Description:
A company is looking for a Nurse Navigator 1 - Site Disease to
work remotely in a full-time capacity.
Key Responsibilities
Triage new patient appointments and ensure timely scheduling
Educate patients on treatment plans and support them throughout the
care continuum
Collaborate with a multidisciplinary team to improve patient care
and address barriers to treatment
Required Qualifications
Bachelor's Degree in Nursing
Registered Nurse Licensing (RN)
Minimum of 1 year of oncology experience
Ability to assess patient needs and facilitate appropriate
referrals
Experience with Electronic Medical Record systems
