The Care Coordinator Supervisor is responsible for providing
support, guidance, and training to the Care Coordinator team,
including assisting with management of health center questions and
conflicts, and monitoring staff's productivity and performance. The
Care Coordinator Supervisor is also responsible for providing
comprehensive care management and care coordination for patients
particularly those with serious, complex, or chronic health
problems or psychosocial issues.
Responsibilities:
- Provide initial and ongoing departmental orientation and
education to care coordinators.Assist in maintaining staff
schedules, coverage plans and assignments.
- Monitor staff productivity related to departmental goals and
organizational initiatives.
- Assist in collecting, preparing, submitting, and reporting on
metrics for monthly reports.
- Respond to escalated patient complaints and provide proper
follow-up with patient and staff in a timely manner; escalate to
Director of Case Management, when necessary.
- Manage and respond appropriately to health center questions and
conflicts.
- Provide feedback on staff performance to Director Case
Management
- Perform quality assurance audits to ensure all processes and
protocols are followed consistently.
- Participate in the creation and implementation of policies and
procedures used for Care Coordination.
- Process referrals for follow-up and closure, as needed.
- Collaborate and partner with care team to identify patient
treatment goals and barriers to care.
- Educate patients on treatments, procedures, medications, and
continuing care needs chronic care management.
- Collaborate with close of clinical care gaps.
- Serve as the contact point and informational resource for
patients, care team and family/caregiver(s).
- Provides reconciliation of Care Coordination monthly
productivity reports to supervisor
- Site specialist for Care Coordination addressing system issues
and ensuring compliant workflows are being followed.
- Promote and monitor adherence to a care plan, developed in
coordination with the care team, patient, and
family/caregiver(s).
- Assess barriers and connect patients to relevant community
resources, with the goal of enhancing patient health and well-being
and increasing patient satisfaction.
- Document pertinent care management information (e.g., plans,
interventions, goals).
- Effectively maintain and protect the confidentiality of all
medical records and patient information.
- Collaborates with supervisor to align with patient centered
medical home practices,
- Timecard management and supports supervisor for
maintaining.Qualifications:
- Associate degree or an equivalent combination of education and
relevant care coordination experience.
- A minimum of 3 years of care coordination experience in an
outpatient clinical setting.
- At least 1 year experience in a lead capacity.
- Experience in chronic disease management, case management,
utilization management, and acute care or Certificate of Completion
in Care Management.
- Knowledge of medical terminology and care coordination.
- Knowledge of the HIPAA Privacy and Security Rule.
- Bilingual in English and Spanish.
- Computer proficiency, particularly with Electronic
Health/Medical Record systems and the Microsoft Office suite,
including Outlook, Excel, PowerPoint, and Word.Preferred
Qualifications:
- Bachelor's degree in social work, human services, counseling,

for this role $25.41 to $35.57 on an hourly basis.TrueCare is

pay range), will be determined based on factors such as education,
knowledge, skills, and experience. In addition to those factors, we
believe in the importance of pay equity and consider the internal
equity of our current team members when determining any final
offer. Please keep in mind that the range mentioned above is the
full base salary range for the role. Hiring at the maximum of the
range would not be typical to allow for future and continued salary
growth. We also offer generous benefits and retirement
